LEADERS APPLICATION FORM

The following form is only to be filled out if you are a leader who has already been accepted onto the leadership team of a CRU Camp.

All leaders need to complete the CRU Leaders Application Form even if you have led on a camp recently. It takes about 20 minutes to fill in, but can be saved on your system which will significantly reduce the time it takes to complete next time you lead.

Returning leaders who have completed this electronic form previously should open their saved file, edit it, and follow the instructions from point 2 below. Please contact us if you cannot locate the file on your system and we may be able to locate it for you.

INSTRUCTIONS

1. Download the Leadership Form above.
2. Fill it in and save it (choosing 'File' and 'Save As') in the format 'lastname_firstname_campname'.
3. Once this form is completed, it's very important that you do BOTH of the below:
         A. Email the saved document to crucamps@crusaders.edu.au AND to your camp director.
         B. Fill in and print the Working with Children Check. Hand sign it, scan / photograph it along with your drivers license or passport (and any relevant qualifications such as 1st aid), and send it all to the CRU Camps office via:

4. Pay online via www.payway.com.au (Biller code: 152280, ID:Invoice number or lastname_firstname if no invoice has been sent to you)
